Efficacy of Protopanaxadiol
Protopanaxadiol has special efficacy in preventing cancer and aiding post-cancer surgery recovery. As an active ingredient of ginseng, protopanaxadiol also enhances immunity, combats fatigue, and alleviates depression.
The ginsenosides commonly referred to as having antitumor activity are ginsenoside Rg3 and ginsenoside Rh2. Modern research indicates that after oral ingestion, ginsenoside Rg3 and ginsenoside Rh2 undergo comprehensive action by gastric acid, intestinal bacteria, and other bodily mechanisms, ultimately converting into protopanaxadiol.
Recent studies have shown that protopanaxadiol is the actual substance responsible for the anticancer effects of ginsenosides such as Rg3 and Rh2. Ginsenoside Rh2 can be obtained by hydrolyzing the terminal glucose at the 3-position of ginsenoside Rg3, and further hydrolyzing the last glucose at the 3-position of ginsenoside Rh2 ultimately yields [(20S)-Protopanaxadiol].

In 1983, Japanese scholar Kitagawa Koichi first isolated 20(R)-ginsenoside Rg3 and 20(S)-ginsenoside Rg3 from Korean red ginseng. Ginsenoside Rh2 is a degradation product of 20(R)-ginsenoside Rg3. Studies have shown that the unique ginsenoside Rh2 in red ginseng is almost absent in white ginseng, suggesting that ginsenosides Rg3 and Rh2 are secondary saponins formed during the process of steaming white ginseng into red ginseng. However, the content of Rh2 in red ginseng is also very low, with an extraction yield of less than one in a hundred thousand, meaning that at most 1mg can be obtained from 100g of red ginseng; the content of Rg3 in white ginseng generally does not exceed two in ten thousand. Assuming that all Rh2 converts to protopanaxadiol, approximately 10 grams of ginseng would be equivalent to 10mg of protopanaxadiol.
